Job overview

We have a vacancy for a Vascular Nurse Specialist based within Vascular Services at Northern General Hospital.

The Vascular Clinical Nurse Specialist is a specialist resource for health care professionals, patients and carers within the agreed scope of practice, whilst taking responsibility for continuity of care. This includes independently examining, assessing and diagnosing patients with highly
complex or chronic conditions, nonmedical requesting of investigations, planning and implementing care; within nurse led clinics and comprehensive follow up services across the regional services.

The autonomous assessment and review of inpatients at Rotherham and Barnsley Hospitals referred into the vascular service, ensuring a timely transfer of patients who have time critical conditions, and/or providing specialist advice locally.

To provide the Vascular Rapid Access Clinic with autonomous Clinical Nurse Specialist assessment of patients with Chronic Limb Threatening Ischaemia, arranging same day MRA imaging/admission as required as part of the Critical limb pathway.

The Senior CNS will lead the Vascular Clinical Nurse Specialist team, and develop the Vascular service, in particular the vascular nurse specialist service and pathways, in conjunction with other clinical and admin members of the service.

You will work alongside vascular consultants, providing specialist clinical support, wound care advice and treatment.

Main duties of the job

Undertake structured training and assessment programmes enabling the post holder to undertake the full duties of the role.

To lead, support, manage and facilitate the VNS team in the provision of appropriate, high quality care to patients with vascular disease.

Lead the vascular Nurse specialist team and develop the service to ensure skills and expertise are utilised in all vascular pathways.

To deliver a holistic, high-quality service that reflects current clinical practice and critical thinking utilising the most appropriate models of care and evidence based practice.

To undertake physical examination and assessment of patients and develop a holistic individualised management plan.

Work autonomously in both the outpatient and outreach inpatient environments.

Arrange direct emergency admissions of patients as required.

Accept and triage referrals to the VNS service and to discharge patients when assessed as suitable to be managed in the community.

Ensure referrals are placed on the correct pathway in a timely
fashion.

Essential criteria

1. RGN Level one
2. NMC registered
3. To have completed the national practice assessor training or equivalent teaching and assessing qualification for those with ENB998.
4. Continuous professional development
5. First degree or evidence of working towards one (demonstrating graduate attributes)
6. Evidence of Level 7 study

Experience

Essential criteria

7. Extensive post registration experience In an acute NHS setting
8. Existing band 6 experience as a Vascular CNS
9. Able to work effectively as part of a multidisciplinary team, crossing professional and organisational boundaries
10. Ability to lead, motivate and develop a professional team
11. Specialist training relevant to working in a Vascular Clinical Nurse Specialist
12. Ability to work within defined timescales

Skills and Knowledge

13. Evidence of relevant post basic training and continuous professional development
