Successful applicants will join South Sefton Primary Care Networks Mental health hub, with the right skills and values to be an exceptional early years practitioner and care coordinator. Leading on and developing a nurturing, safe and stimulating creche modelling a trauma sensitivity, working across a range of community sites.

If you are passionate about transforming lives and making a real impact on peoples health and well-being, this is an exciting opportunity to work with primary care to engage and support families living in Bootle, Seaforth, Litherland, Crosby and Maghull. As an ACE early years practitioner and care coordinator you will work as part of the ACE health and wellbeing team to champion and deliver the adult ACEs recovery toolkit programme. Using creative activities and group work, providing psycho education and pro social modelling to develop participants resilience giving them opportunity to experience the healing of relational support.

Maintaining and developing quality Creche provision sensitive to diverse needs of parents and children affected by adverse childhood experiences.

Working in partnership with families to support parental understanding of their role in their childs development, modelling Trauma sensitive, Nurturing, high quality Early education within the Creche.

Identify levels of risk, vulnerability and needs and a proven ability to recognise and manage risk.

Provide a trauma sensitive, empowering person centred approach.

Sensitively challenge children, families and individuals.

implement safeguarding procedures and the escalation policy

Recognise factors impacting on families such as domestic violence, abuse, mental health difficulties, substance misuse, poverty, early years development, finance and housing issues.

Work well in a multi-disciplinary team

Organisation and time management including planning, prioritising, preparation, follow-up and record keeping.

Ensuring data integrity, confidentiality, and security to safeguard all personal identifiable information.

Excellent interpersonal and communication skills and ability to interact professionally with people from diverse cultural, racial, ethnic, gender, and socioeconomic backgrounds.

Preparing resources for Groups and Creche.

South Sefton PCN is a large Primary Care Network, covering the four localities in south Sefton with a population of around 155,000 residents

PCNs are groups of GP practices working with a wide range of partners to improve the health of their population. Key to this purpose is strengthening general practice provision through working together, where it makes sense to do so and working as part of integrated care teams which work collaboratively for and with local people in each locality.

South Sefton PCN Mental health Hub provides proactive Primary care level mental health and psychological support to residents Via the Associate Psychological practitioner offer. Alongside empowering resilience building support, Psycho education and coaching both group and 1:1 via the ACE health and wellbeing team offer.

The Care Coordinator is a key member of South Sefton Primary Care Network (PCN) supporting the delivery of high-quality patient care by strengthening the offer of our multi-disciplinary team.

Our ACE (Adverse childhood experiences) Early Years Practitioner and care coordinator will provide a safe, nurturing, and responsive environment for young children in a trauma-sensitive crche setting. Support childrens emotional, social, and developmental needs while being mindful of the impact generational trauma may have on their behaviour and well-being. Work closely in a calm predictable manner with children, families, and colleagues to create a supportive space where children can feel secure, valued, and engaged in age-appropriate play. Tailor interactions and activities to be trauma-informed and linked to the outcomes of the ACE recovery toolkit programme, to support each child's overall well-being and development and reduce the likelihood of ACEs. Contribute to the ongoing development of trauma-sensitive practices within the setting, ensuring that all care, interactions and approaches are sensitive to the diverse needs of parents and children affected by adverse childhood experiences. Building open, non-judgmental, and supportive relationship with parents and providing them with updates on their childs time in creche and activities they may want to use at home.

Facilitate the ACE recovery toolkit programme as part of a team of trained facilitators working alongside adults to coach and motivate them through multiple sessions, supporting them to identify their needs, set goals, and help them to implement their personalised health and care plan. Activities are delivered through appropriate models and frameworks to engage, empower and enable residents to take ownership of their own solutions and develop resilience.

Provide a trusted point of contact for participants and work as part of a team in providing care coordination. P romote Self-care and health messaging and engagement with appropriate healthcare and the universal offer through the community, voluntary and faith sector organisations. Duties will include a range of administrative tasks. The ideal candidate will be an organised individual with excellent interpersonal skills.

Job Responsibilities

Work With GPs And The PCN Team To Identify And Engage a Case Load Of Clients That Would Benefit From Very Early Help To Proactively Support Individual, Family And Household Wellbeing


* Maintain and develop quality Creche provision sensitive to the diverse needs of parents and children affected by adverse childhood experiences. Work in partnership with families to support parental understanding of their role in their childs development by modelling Trauma sensitive, Nurturing, high quality Early education within the Creche. Increase parental engagement to strengthen family relationships and support childrens wellbeing and healthy development.
* Deliver targeted activity in groups providing a holistic approach to address the root causes utilising the Rockpool Recovery toolkit programme.
* Support people and their Carers, providing coordination and navigation across health and care services, collaborating closely with the PCNs Mental health hub and ACE health and wellbeing team, GPs, practices, social prescribers, pharmacy technicians, and other primary care professionals. To work as part of a team to progress clients to achieve positive outcomes based on a solution focussed empowering approach.
* Signpost participants to activities within the community to build resilience within the family. Support family members to develop skills to enable them to carry out household tasks such as budgeting, supporting health and hygiene, increasing parental confidence and capacity.
* Help people to manage their needs by answering queries, making, and managing appointments, ensuring that people have good quality written or verbal information to help them make choices about their care and maintain accurate secure records using PCN systems.
* Assess and identify levels of risk, vulnerability and safeguarding using prescribed assessment tools, and escalating to GPs as necessary. Following safeguarding procedures.
* Support in providing assessments Via Phone and face to face in the community or on home visits to engage and assess participants for the ACE programme and ensure that Sefton residents are safeguarded within the community.
* Work co-operatively with colleagues and partners to share information, with consent and within GDPR, ensuring a joined-up approach.

10 Ensure Quality Assurance, legislation, regulations, policies and procedures are central to all good practice.

11 Engage in the development of the service and CPD through PDR, training team/service meetings, and supporting raising awareness sessions across the partnership on key and emerging issues.

1 12 Utilise appropriate marketing techniques to promote awareness of the service and represent the service at events as required. including contributing to COMMs / social media.

13 Ensure that client information data is lawfully gathered, accurate, up to date and only divulged in accordance with GDPR.

SPECIAL CONDITIONS

The post holder will be expected to move between locality delivery points depending on the needs of the service.
