Primary Nurse

As a member of staff within the Short Breaks Service you will be expected to work in partnership with all families, agencies and service providers to assess, implement and evaluate the health and social care needs of the client while offering a fun, safe and secure environment and ensuring that the person is at the centre of all you do.
To work within this highly engaging integrated service you will be required to be a dynamic, well‑motivated individual who possesses excellent communication skills, is willing to learn and develop in the role and has the skills and knowledge to support adults with Learning Disabilities and associated complex needs.
Main duties of the job
* Participate in providing Short Breaks for clients with complex disabilities through a health care model.
* Work within the requirements of the policies and guidelines of the service, ensuring that clients are supported and assisted with all aspects of daily living, meeting their identified health needs and facilitating an individualised approach to leisure and activities.
* Provide nursing care for adults with learning disabilities and complex health needs, working within the limits of one’s own competence, responsibility and accountability.

Person Specification
Skills and abilities
* Effectively communicate with individuals, carers, families, team and service members and other agencies, including summarising and communicating complex information.
* Work flexibly and respond to change.
* Approachable and accessible to colleagues and across the organisation.
* Reflect upon own practice and performance through effective use of professional and operational supervision and appraisal.
* Work autonomously and collaboratively within a team, with good personal organisational skills and ability to work under pressure.
* Prioritise and work to deadlines in a busy environment.
* Take a proactive approach to process and service improvement.
* Effective written and oral communication skills, including use of IT systems.
Qualifications / Experience / Knowledge / Skills
* Recognised Nursing Qualification – RNLD preferable.
* Experience engaging and communicating effectively with people at all levels.
* Experience working as an effective and responsible member of a team.
* Knowledge of current issues and best practice in Learning Disability Nursing.
