Social prescribing empowers people by addressing the wider determinants of health such as debt, housing issues, unemployment, loneliness, caring responsibilities and physical inactivity through personalised, non-medical support.

Weekly hours will be 25 hours a week and the needs of the business dictates that one of these days will be a Monday.
Main duties of the job
As a Link Worker, you will:
* Receivereferrals from the clinical team and work directly with patients toexplore what matters to them.
* Co-producepersonalised care and support plans.
* Connectindividuals to community groups, voluntary services and statutoryorganisations.
* Supportpatients through Cancer Care Reviews and act as liaison with their GP.
* Promoteindependence, resilience and self-management.
* Workas part of the multidisciplinary team (MDT), providing feedback andraising awareness of available community services.
* Developstrong relationships with local organisations and community groups,helping to strengthen and sustain local provision.
* Maintainaccurate records and demonstrate the impact of the service through datacollection and case studies.
This role requires someone who can manage their own caseloadeffectively, recognise when to elevate concerns appropriately (includingsafeguarding and mental health needs), and build trusted relationships withboth patients and colleagues.

Job responsibilities
MAIN DUTIES AND RESPONSIBILITIES:
Receivereferrals from the clinical team at the surgery you are based
Discussthe persons needs with them, based on guidance from the referrer, and identifya range of options that could assist the person to improve their independenceand health and wellbeing
Supportpatients with Cancer Care Reviews ensuring individual needs are met and to actas liaison with the patients GP.
Strengthencommunity and personal resilience, focusing on what matters to me and takinga holistic approach with each individual case
Co-producea simple personalised care and support plan to improve health & wellbeing,introducing or reconnecting people to community groups and statutory services
Manageand prioritise own caseload, in accordance with the needs
Havea strong awareness and understanding of when it is appropriate or necessary torefer people back to other health professionals / agencies, when the personsneeds are beyond the scope of the Link Worker role e.g. when there is a mentalhealth need requiring a qualified practitioner
Workin partnership with voluntary and statutory organisations
Understandthe barriers and opportunities for people to self-manage their conditions inthe community
Havea role in educating clinical and non-clinical staff within their PCN multi-disciplinary teams on what other services are available within the communityand how and when people can access them.
Key Tasks:
Promotesocial prescribing, its role in self-management and the wider determinants ofhealth
Aspartof the surgerys MDT provide information and feedback on socialprescribing on request
Workwith the other link workers in our PCN to share knowledge, learn, and improve
Beproactive in encouraging self-referrals and connecting with all localcommunities, particularly those communities that statutory agencies may findhard to reach
Bea friendly source of information about wellbeing and prevention approaches.Help people identify the wider issues that impact on their health andwellbeing, such as debt, poor housing, being unemployed, loneliness and caringresponsibilities
Workwith the person, their families and carers and consider how they can besupported through social prescribing
Helppeople maintain or regain independence through living skills, adaptations,enablement approaches and simple safeguards
Seekadvice and support from relevant GPs to discuss people-related concerns (e.abuse, domestic violence and support with mental health), referring the personback to the GP or other suitable health professional if required
TheLink Worker will be expected to keep accurate and up- to- date records onrelevant health systems
TheLink Worker will gather record and collate data, including case studies, in aprescribed format to demonstrate the impact of the service
Undertakeany tasks consistent with the level of the post and the scope of the role,ensuring that work is delivered in a timely and effective manner.
Supportthe care coordinating team when required covering sickness and annual leave
Person Specification
Qualifications
* GCSE grade 4 and above in English and Maths
* Qualified to NVQ level 2 in Health and Social Care
Experience
* Experience of dealing with vulnerable people
* Experience of supporting people on a one to one basis
* You will be organised, proactive and able to prioritise effectively
* Experience of working in primary care
* Experience of working in GP practice
Knowledge and Skills
* Full clean driving licence
* Have excellent listening and communication skills
* Ability to provide quality care
* Can work effectively alone and as part of a team
* Efficient and organised in approach to work
* Confident in engaging with external services
* Experience in social prescribing, health and well-being support, community development, or similar roles would be advantageous but is not essential.
* Basic IT skills to include Microsoft 365
