The flow co-ordinator role in mental health will encompass that of expert clinician and decision maker, educator, and leader. It will be a pivotal role to assist, support and promote seamless patient journey up to and including discharge, promoting a positive patient experience. The caseload will vary daily as it will be all East Lothian mental health patients in any acute setting, such as REH and SJH inpatients, but also admissions to RIE and WGH. The postholder will be required to rapidly analyse assessments pertinent to each patient and synthesise a cohesive discharge plan. This may include but is not limited to Psychiatry, Nursing, Occupational Therapy, and Adult Social Work assessments. To provide senior clinical support to the Service manager through day-to-day co-ordination of the Mental Health Team with specific focus on all matters to do with patient flow. To provide highly specialist advice to healthcare professionals (IHTT, CMHT, CWIC, PTS, Psychiatry), other agencies, carers, clients and relatives.

To support the Clinical Nurse Manager in the enhancement and strategic development of the existing service as well as the development of any new services based on the needs of the local community. Will work alongside the Team Leads to deliver the services. The post holder will also be required to deputise into a leadership and management role in instances of Team Lead absence.

Responsible for working closely with the relevant clinical teams to develop and deliver programmes of care which includes assessment, planning, implementation and ongoing evaluation of care needs for patients primarily in the domiciliary setting over the 24-hour period, to individuals within the East Lothian community within the available resources. This is often in partnership with the Primary and Secondary Care Team, Unscheduled Care Service and other agencies involved in delivery of care in the community.

To act as the key clinical lead when working with health and local authority colleagues to prevent hospital admissions and to ensure early and effective hospital discharge. To take a key role in leading and developing the multi-agency team to improve practice and work collaboratively, implementing and evaluating evidence-based standards, guidelines and policies.

This post requires the post holder to have a PVG Scheme membership/record. If the post holder is not a current PVG member for the required regulatory group (i.e. child and/or adult) then an application will need to be made to Disclosure Scotland and deemed satisfactory before they can begin in post.

If the post is assessed as a "regulated role", your appointment will be subject to joining the Protecting Vulnerable Groups (PVG) Scheme.
