Liquid Personnel is recruiting an Adult Social Worker to join its client’s Integrated Network team, based in Bridgend. This is an exciting time to join our Integrated Cluster Network Teams. The Cluster Team work and support to develop a team around the person and their family or those important to them. The people we support can have long term complex challenges around their physical health, mental health and emotional well-being. It works with both older and younger adults. We aim to provide a holistic support to allow to live their lives as they wish to and ensure voice, choice, independence and control in their lives. What will your responsibilities be? Undertake strengths‑based, person‑centred assessments under the Social Services and Well‑being (Wales) Act 2014, identifying eligible care and support needs for adults and carers Develop, implement and review care and support plans that promote independence, well‑being and community inclusion Complete assessments relating to hospital admission, discharge planning, community living, and mental health needs Support individuals to access appropriate services, including domiciliary care, reablement, residential care, equipment, adaptations and direct payments Qualifications and Experience: To be successful in this role you must have. Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, or CQSW. Registration: Must be registered with Social Care Wales. Experience: 2 years post-qualified experience working in a statutory setting. Legislation Knowledge: Strong understanding of the Mental Health Act 1983, Mental Capacity Act 2005, Social Services and Well‑being (Wales) Act 2014, safeguarding procedures, and the Mental Health (Wales) Measure 2010. Skills: Excellent assessment, communication, and risk‑management skills, with the ability to work effectively in a fast‑paced multidisciplinary environment.
