The role includes: Supporting individuals to leave hospital as soon as is practicably possible and to safely transition back to living at home as independently as possible. Visiting individuals in their own homes and providing appropriate care and support as defined in the Care Plan. This will include aspects of personal care, administering medication, monitoring of vital signs and exercise. The aim is to rehabilitate and maximise potential, to support and enable Service Users in a range of activities to promote independent living and monitor their well-being. Providing compassionate and personalised care to individuals promoting their independence and well-being. Assisting with daily living activities, meal assistance and domestic tasks. Communicating effectively with families, healthcare professionals and colleagues to ensure holistic care. The ideal candidate will also need to have: NVQ 2/ Level 2 Diploma in Health and Social care - Mandatory Empathy and understanding of the unique needs of individuals Previous experience in caring for or supporting individuals Excellent communication and interpersonal skills. Ability to work collaboratively in a team and independently. Willingness to undergo relevant training relating to role. Full UK driving license and access to your own vehicle as it is a community based role We are offering contracts with full time guaranteed hours of up to 40hrs per week. Working hours are between 7am-10pm with shifts of 7am-2pm & 4pm-10pm including alternate weekends.
