Job overview
We are currently looking to recruit a full-time Band Mental Health Practitioner (Mental Health Nurse, Occupational Therapist, or Social Worker) to join our small and supportive Community Mental Health Team for older adults, embedded within an exciting Integrated Care model. This is a great opportunity to join our friendly and dynamic multidisciplinary team, with lots of support and CPD opportunities. There is free parking onsite.
We provide patient-focused, evidence-based mental health interventions and assessments for people aged 65 and above, with a range of functional and organic mental health illnesses. You will manage a caseload of service users, delivering clinical skills, therapeutic, and recovery-based interventions. The work is varied and rewarding.
Your primary area of coverage will be East Villages, Cambridge, but you must also remain flexible to support other parts of Cambridge as needed. Regular travel is essential; therefore, successful applicants will need to have a driver’s license and access to a vehicle for work.
The post is based at Sawston Medical Centre. CPFT supports agile working, offering flexibility for work-life balance. Visits to the team are welcome.
Please note: For this role, you will be required to travel independently around the county, meeting strict time deadlines. You must hold a full UK driving license and have use of a vehicle. Public transport is not suitable due to reliability issues and service needs. Please confirm in your application that you meet these criteria.
As a practitioner, you will act as a Care Coordinator for service users and promote carer involvement in care delivery with service user consent.
You will manage a caseload, delivering diverse clinical skills and building therapeutic, recovery-based relationships. We aim to empower service users in their recovery journey, working towards effective discharge and utilizing third-sector support where needed.
The ideal candidate will have a strong team ethic, a sense of accountability, the ability to work under pressure, and a desire for innovation.
