We currently have a vacancy for an experienced and motivated Support Coordinator Complex within the Adult Social Care Hospital Team.

About You...

As a Support Coordinator Complex in our Hospital Team, you will have a minimum of 2 years’ experience working in Adult Social Care. You will be able to respond to general enquiries and crisis situations.

You will be dedicated to upholding high standards of practice, ensuring individuals remain connected to their community and social networks whilst being protected from harm.

As a Support Coordinator Complex you will support our most vulnerable residents to get their lives firmly on track, using a strength-based approach. You will keep the person at the heart of everything you do.

You will be given the time to get to know everyone you work with to change their lives for the better and to be as independent as they can.

Our Support Coordinators Complex have a strong focus on integrated working with our partners to deliver holistic and seamless support when people need it.

About The Team...

The Adult Social Care Hospital Team is a busy and supportive multidisciplinary team.

As part of the team, you will assist the Social Work Team to facilitate timely discharge from hospital, avoid unnecessary hospital admission, reduce dependence on care, support families and enable people to stay within their own communities.

You will have opportunities to be part of the development and growth of the team and will be encouraged to work on projects or focus on areas of interest.

We have regular multidisciplinary forums, meaning our residents can have easy access to health and social care services that talk to each other, to provide holistic support when needed.

We know there can be pressures working in Adult Social Care, so we make sure our staff are well supported, receiving regular supervision and access to training. We have a strong focus on supporting our staff to learn, grow and develop to achieve their potential.

We listen to our social care staff about the best way to deliver our services and encourage feedback and suggestions on how we can do things better together.

Key responsibilities...


* To work as part of a multi-disciplinary team.
* Connecting people and their families to their community/social network. Enabling them to live within their own communities, have a timely discharge from hospital, avoid unnecessary hospital admission and reduce dependence on long term care.
* To support in delivering and maintaining high standards of performance, ensuring continuous development and supporting people to fulfil their roles to maximum effectiveness within allocated resources.
* Take responsibility for own learning, including mandatory and suggested training, and the training of students and identified staff.
* Take an active part in your own supervision and appraisal and to supervise and appraise allocated staff, in accordance with the departmental supervision policy and the council's appraisal policy.
* To take a departmental or team lead within a specified area of work as required.
