We’re the best at what we do and it’s all thanks to talented people like you. As the leading Home Care provider, we take pride in our work - and we’re looking for a caring and dedicated person to join our Chelmsford team as a Care Coordinator, to help us set the standard.
As a Care Coordinator, you’ll help our team grow, support our colleagues, and make a real difference every day. The role includes 20 dedicated hours per week of care coordination, with the option to work additional hours as a Care Expert, up to full-time hours in total.
You will ensure customers receive safe, consistent and person-centred care in their own homes, while supporting our Care Experts through efficient scheduling and clear communication.
Key Responsibilities
* Create and maintain efficient rotas, ensuring care is delivered on time and in accordance with customer care plans.
* Work with the Registered Manager to ensure appropriate staffing levels and skill mix.
* Arrange cover for staff holidays and absences.
* Process new customer referrals and changes to care needs promptly.
* Support Care Experts by monitoring workloads and promoting work–life balance.
* Match Care Experts to customers based on skills, experience, availability and preferences.
* Be an active participant in on-call, following training and competency sign-off.
* Maintain accurate digital and written records, including travel and incident reporting.
* Ensure confidentiality and adherence to company policies at all times.
* Work collaboratively with colleagues and external professionals to deliver exceptional homecare services.

About You
* Excellent organisational and communication skills
* Calm under pressure with strong problem-solving abilities
* Confident using digital systems
* Team-focused and committed to delivering exceptional care
* Experience in scheduling or care coordination is beneficial but not essential
* 20 hours per week coordinating
* Additional hours available as a Care Expert up to full-time hours available
