Job Responsibilities
* Provide high‑quality, evidence‑based nursing care to patients registered at the practice.
* Manage patients with long‑term conditions such as diabetes, asthma, COPD and hypertension.
* Undertake chronic disease reviews, medication checks and titration in accordance with local and national guidelines.
* Perform cervical cytology, childhood and adult immunisations and travel vaccinations.
* Carry out treatment‑room duties, including phlebotomy and administration of therapeutic injections (e.g. B12, Zoladex, Prostap).
* Provide health promotion and lifestyle advice on smoking cessation, weight management, diet and exercise.
* Support women’s and men’s health services as required.
* Maintain high standards of infection prevention and control in all clinical practice.
* Work collaboratively with GPs, ANPs, pharmacists, HCAs and administrative staff to ensure coordinated patient care.
* Participate actively in clinical meetings, audits, quality improvement projects and in the delivery of QOF, IIF and enhanced services.
* Maintain accurate, contemporaneous clinical documentation in accordance with NMC standards using EMIS Web and Ardens templates.
* Support effective handover and communication within the team.
Health Promotion & Public Health
* Deliver patient education to promote self‑care and healthy lifestyle choices.
* Participate in national and local health campaigns such as flu and NHS Health Checks.
* Identify and respond to safeguarding concerns for both adults and children in line with practice and statutory procedures.
Teamwork & Communication
* Work collaboratively with GPs, ANPs, pharmacists, HCAs and admin colleagues to ensure a coordinated approach to patient care.
* Maintain accurate clinical documentation and support effective handover.
Professional Development
* Maintain NMC registration and comply with revalidation requirements.
* Complete all mandatory and role‑specific training, including BLS, infection control and safeguarding (Level 3).
* Participate in annual appraisals, reflective practice and clinical supervision.
* Engage in continuous professional development to maintain and enhance clinical skills.
Qualifications and Experience
* Essential:
o Registered Nurse (RGN) with current NMC registration.
o Experience working within a primary care or community nursing setting.
o Competent in chronic disease management and general treatment‑room duties.
o Up‑to‑date training in immunisations, cervical cytology and basic life support.
o Familiarity with EMIS Web, Ardens templates and QOF processes.
o Excellent communication, interpersonal, and organisational skills.
o Commitment to ongoing professional development and teamwork.
* Desirable:
o Additional qualifications in chronic disease management (e.g. asthma, COPD, diabetes).
o Experience delivering travel health advice and vaccinations.
o Mentorship or teaching qualification to support HCAs or trainee nurses.
