We are seeking an enthusiastic and motivated individual for an exciting opportunity as a Care Navigator. You will be responsible for undertaking a wide variety of medical receptionist activities and providing support for the GP practice.

Main duties of the job
To receive, assist and direct patients in accessing theappropriate service or healthcare professional in a courteous, efficient andeffective way.
To provide general assistance to the Practice team andproject a positive and friendly image to patients and other visitors, either inperson or via the telephone.

Job responsibilities
Some of the job responsibilities include:
Process appointment requests from patients bytelephone and in person
Deal with visits requests
Respond appropriately to patients queries
Accurate recording and timely passing on ofmessages, ensuring audit trail is maintained
Registrations of new patients computer dataentry and medical records
Process patients change of address computerdata and medical records (have knowledge of Practice area)
Process repeat prescription request inaccordance with Practice guidelines
