Job Summary
We are seeking a compassionate and dedicated Support Worker to join our team. The successful candidate will play a vital role in providing assistance and support to individuals, particularly children, in various settings. Duties
* Assist individuals with daily living activities, ensuring their comfort and safety.
* Develop and implement personalised care plans tailored to the specific needs of each individual.
* Communicate effectively with clients, families, and other professionals to ensure continuity of care.
* Provide support in childcare settings, fostering a nurturing environment for children.
* Drive clients to appointments or activities as needed, ensuring safe transportation.
* Maintain accurate records of care provided and any changes in the individual's condition.
* Collaborate with multidisciplinary teams to enhance service delivery and client outcomes.
Skills
* Strong communication skills, both verbal and written, to engage effectively with clients and their families.
* A valid driving licence is preferred for transporting clients as required.
* Proficiency in IT skills for maintaining records and utilising care management software.
* Experience in developing and implementing care plans that meet individual needs.
* Background in childcare or experience working with children is highly desirable.
* Ability to work empathetically and patiently with individuals from diverse backgrounds.
