In particular, we are looking for someone with an interest in Frailty at the Front door with the aim of improving the assessment and management of older people in ED and their pathway through to appropriate wards for ongoing management. This will be done in conjunction with an ongoing Gold Quality Improvement plan looking at changing services to meet the needs of the older frail population of Somerset.
