Overview
We are seeking caring, motivated and compassionate individuals to join our Dementia Assessment Unit within the Specialist Older People's Inpatient Services. This is a rewarding opportunity to support older adults living with dementia who require specialist assessment, therapeutic input, and person‑centred care.
Responsibilities
* Personal Care: Assist with hygiene, mobility, nutrition, hydration, and emotional wellbeing while promoting dignity and independence.
* Clinical Support: Help with observations, monitor wellbeing, report changes, support emergencies and therapeutic interventions, and maintain a calm, safe environment.
* End‑of‑Life Care: Provide dignified support, work with families and the team to ensure comfort and respect.
* Admissions & Discharges: Assist with admissions, transfers, discharges, and accurate documentation.
* Communication & Documentation: Engage with patients, families, and colleagues professionally; record information promptly.
* Teamwork & Patient Engagement: Collaborate with staff, support activities and therapeutic sessions, encourage engagement.
* Safety & Flexibility: Promote safety, follow safeguarding policies, report concerns, and work across wards as needed.
