Transfer of Care Facilitator – Band 4
An exciting opportunity to join the Transfer of Care Integrated Team as a Transfer of Care Facilitator. We are a team based at the hospital that supports patients discharges from the acute wards and the community bed facility.
Responsibilities
* Support wards and attend board rounds to ensure safe discharges.
* Assess patients for Home First Discharges.
* Provide patient information for the No Criteria to reside data.
* Complete equipment assessments to ensure safe discharges.
* Update electronic data systems daily with discharge plans.
* Work Monday to Sunday 8:00‑16:00.
