What you will do: Attend daily referral meetings and contribute to multi-disciplinary reviews, ensuring the best outcomes for service users. Conduct comprehensive assessments of need, manage risks, and develop recovery plans tailored to individual clients. Manage a caseload effectively, offering both one-on-one sessions and facilitating group interventions. Collaborate with various services such as housing, employment, healthcare, and more to provide holistic support. Promote and signpost services to ensure comprehensive care for service users. Maintain accurate and professional records, adhering to data protection and GDPR requirements. Support clients as they transition from the detention centre back to the community, preparing them for release or deportation. Assist in service improvement initiatives and meet key performance indicators set by the Health and Wellbeing Manager.
