People Who Care provide person-centred assistance and support to people living with a disability, helping them to achieve their goals and remain as independent as possible. This role will also provide services for our Veterans Program.
To succeed in this role:
* You will need to be caring, patient, responsible and experienced in working with individuals with specific needs. Our clients may be experiencing psychosocial, physical and intellectual challenges.
* You may be asked to support our clients with mobility, personal care, preparation of meals, domestic tasks, accessing their community and other aspects of their daily life.
* Being flexible and proactive in your approach will help you to contribute positively to their wellbeing.
Responsibilities:
* Support individuals with a range of activities, such as assisting to maintain their homes (domestic assistance), accessing the community and attending appointments (social support).
* Support individuals to achieve and meet their goals as specified in their NDIS plan (or Veterans HCP Plan).
* Provide regular case notes and communicate with the Service Coordinator about the health, well-being and progress of the individual.
* Work collaboratively with the individual and the disability services team, and provide support that best meet the individual’s needs.
What you will need:
* Empathy and understanding of people with disabilities and their family members/carers.
* Strong communication and interpersonal skills.
* Ability to work with individuals from a range of social, financial and cultural backgrounds.
* Ability and willingness to undertake a wide range of tasks.
* Ability to cope with changes in work demands, to think outside the box and problem solve.
* First Aid Certificate (or willing to obtain).
* Willingness to learn and competency in using technology (internet, email and smartphone).
* Current NDIS Worker Screening Check.
* Current National Police Clearance (not older than 6 months).
* Experience assisting people living with high care needs.
* Certificate III or higher in Disability, Aged Care or Community Services.
