Overview

ARRS CARE COORDINATOR - Full-Time (37.5 hours / week). Are you passionate about improving patient care and making a real difference in people’s lives? We are looking for a Care Coordinator to join our GP practice, helping patients navigate their healthcare journey with ease. As a key part of our multidisciplinary team, you will work closely with GPs, pharmacists, nurses, social prescribers, Age UK, Safeguarding and Palliative Care Teams, to ensure patients receive the right care at the right time. You will coordinate referrals, assist with care planning, and support patients with long‑term conditions and complex health and care needs; you may need to see patients face to face to reduce hospital admissions and enhance wellbeing.

Responsibilities

* Support patients with mental health needs, dementia, learning disabilities, cancer, and general wellbeing concerns by managing caseloads and coordinating care across Primary, Secondary, and Community services.
* Refer patients to appropriate health professionals, support external referrals, and ensure patients benefit from available services.
* Provide administrative support, assist with QOF and DES/LES work, coordinate care plans, engage with vulnerable patient groups and their carers, and organise key meetings to facilitate effective patient care.
* Fulfil other administrative duties as required to support the smooth running of the practice.
* Support safeguarding activities, information governance, and clinical governance as part of safeguarding duties, including keeping policies up to date and training staff.
* Facilitate an effective safeguarding MDT for children and vulnerable adults and maintain a vulnerable patient register.
* Promote effective collaboration with local safeguarding partners and ensure timely sharing of information when safeguarding concerns arise.
* Undertake audits, report findings, and respond to statutory information requests related to safeguarding case reviews.
* Promote understanding of local safeguarding referral pathways and attend safeguarding meetings and trainings as required.
* Identify patients who meet QOF/LES/DES criteria, perform assessments (telephone/video/face to face), and coordinate referrals and care plans with GP and PCN teams. Address escalation of care plans and end‑of‑life preferences where appropriate.
* Review and code hospital discharge summaries, contact patients within 48 hours of discharge, signpost to relevant services, and follow up as needed; maintain records and support hospital discharge processes.


Qualifications & Experience

Essential

* Experience of working within a healthcare team
* Experience of dealing with vulnerable patients
* GCSE grade A–C in English and Maths

Desirable

* Experience of working in primary care and in a GP practice
* Experience of utilising SystmOne
* Knowledge of QOF and DES/LES requirements
* NVQ level 2 in Health and Social Care
