Overview
We have an exciting opportunity for a rehabilitation-focused Senior Occupational Therapist to join our Inpatient Rehabilitation Service on Welney Ward, Princess of Wales Hospital, Ely. This position works with patients on inpatient wards to support them out of hospital, offering holistic clinical assessment, rehabilitation, discharge planning and specialist therapy knowledge. Successful applicants will support patient flow through pathways and provide clinical skills and knowledge to the MDT. The role offers flexibility, variety and involvement in the transformation of our inpatient rehabilitation service, supported by a Band 7 clinical lead.

Responsibilities
* To be responsible for own caseload and undertake assessment of patients with complex and multiple pathologies using specialist clinical reasoning skills.
* To identify patient needs, agree goals and provide appropriate therapy interventions, provide adaptive equipment, rehabilitation programmes, and make recommendations for minor adaptations in line with current legislation.
* To keep the patient at the centre of care, ensuring patient and carer participation in decision making. To provide support and education to peers, new staff, non-registered staff and students.
* To be accountable for a delegated caseload held by Band 5 Therapists and Assistant Practitioners.
* To prioritise all patients’ tasks according to clinical need and to allocate or signpost appropriately. To provide a high standard of care.
* To work in various community settings within CPFT as required, completing access and home visits.
* To organise and manage own time, delegating work appropriately. Conduct risk assessments and health and safety assessments and report any changes/newly identified risks.
* To demonstrate the importance of gaining patient and carer consent in all interventions.
* To effectively communicate verbally with patients, carers and colleagues, including patients with communication difficulties (hearing loss, visual impairment, depression, speech problems, cognitive impairment, behavioral problems, and pain).
* To actively manage positive relationships with other agencies involved in the patient’s pathway. To work effectively within the MDT, attending MDT meetings, case reviews and family meetings.
* To ensure accurate and timely written records in line with Trust policy and HCPC guidance, reporting issues as appropriate.
* To provide specialist advice to MDT members, service users and others regarding changes to care plans and patient progress.
* To work within Trust and COT guidelines and maintain knowledge of national and local standards of practice, legal and operational requirements.
* To demonstrate a good understanding of Social Services and Health legislation.
* To support day-to-day planning and delivery of high-quality care.
Qualifications and requirements
* Full UK driving licence and use of a vehicle ( DVLA reciprocal arrangements apply for some overseas applicants; see DVLA website).
* Hold a professional and HCPC registered Occupational Therapist status and be able to work as an autonomous practitioner within the integrated MDT.
* Ability to undertake holistic assessment (including complex presentations and multi-pathologies) using specialist clinical reasoning skills and evidence-based practice.
