Advanced Nurse Practitioner (Care Home and Frailty)
We are seeking an experienced Advanced Nurse Practitioner (ANP) with a passion for care home and frailty services to join our clinical team. You will lead on the assessment, management, and proactive care of frail patients, working collaboratively with GPs, community services, and care home staff to optimise health outcomes and reduce avoidable hospital admissions.
This is a varied and rewarding role, blending autonomous clinical practice with leadership and service development.
Main duties of the job
Provide advanced clinical assessment, diagnosis, and management for patients in care homes and those living with frailty in the community.
Lead proactive frailty reviews and personalised care planning.
Conduct regular ward rounds in allocated care homes, working closely with care home staff and families.
Support early identification of deteriorating residents and initiate appropriate interventions.
Promote safe prescribing and medication reviews in line with best practice.
Contribute to multidisciplinary meetings, service improvement initiatives, and training for staff.
Act as a clinical leader and role model within the practice team.

* Contribute to the development of practice policies and protocols related to frailty and care home services.
Education & Training
* Provide training and support to care home staff in clinical skills, frailty management, and recognising deterioration.
* Maintain and develop own clinical expertise through CPD, reflection, and peer review.
* Participate in clinical supervision and appraisal processes.
Professional Responsibilities
* Work within the scope of the NMC Code and relevant advanced practice frameworks.
* Maintain accurate, timely, and complete patient records in line with legal and professional standards.
* Adhere to safeguarding procedures for vulnerable adults and children.
* Uphold confidentiality at all times.
Person Specification
Qualifications
* Registered Nurse with MSc-level Advanced Practice qualification (or equivalent experience).
Experience
* Significant experience in managing complex, frail, and elderly patients.
* Excellent assessment, diagnostic, and decision-making skills.
* Ability to work autonomously and collaboratively within a multidisciplinary team.
* Strong communication skills and a compassionate, patient-centred approach.
* Previous experience working in primary care.
* Knowledge of frailty tools (e.g. Rockwood Clinical Frailty Scale, eFI).
* Experience in service development and quality improvement.
