We are looking for an ambitious, dynamic, motivated, compassionate and enthusiastic Band 5 nurse to join our Community Nursing team in Bedfordshire. This substantive post is for the Mid Beds Community Nursing Team delivering care to people aligned to Biggleswade GP practices and aligned residential homes.

This highly respected team delivers responsive high quality nursing to patients in their own homes.

We are passionate about patient centred care, support our nurses to lead and deliver excellent and compassionate nursing and value our clinical teams as the real heart of the NHS. You will work closely with other members of the MDT to deliver care to patients with complex needs.

You will also have the opportunity to be involved in Urgent Response and Virtual Wards, which is a focus of our teams

Main duties of the job

You will have regular supervision, mentorship and support from within the Primary Care Team and the Trust.

Experience in the field of adult community services is desirable but a willingness to learn and contribute is more important. Newly qualified nurses who are passionate about a career in the community are welcome to apply. There is a preceptorship program and competencies to support your development.

Job responsibilities

You must have a driving license and have access to a vehicle in order to carry out your duties. You will be required to work a 7.5 hour shift pattern covering 7 days a week from 08:00-21:00.

Person Specification


Education

* Registered General Nurse
* Demonstrate evidence-based practice and up-to-date clinical skills.
* Evidence of post-basic professional development
* Post-registration qualification in a relevant clinical field
* Relevant teaching/mentorship qualification or ocommitment to undertake
* Post-registration nursing experience, including community experience


Physical/Mental/Emotional Support

* Ability to mobilise and stands for most of shift; and kneels and crouches to deliver clinical care e.g. dress wounds.
* Ability to concentrate on providing clinical care such as drug calculations, carrying out tests and managing interruptions and unpredictable patient/client /carer behavior
* Occasional distressing or emotional circumstances such as imparting unwelcome news, care of terminally ill or safeguarding issues
* Post holder could work in occasional unpleasant conditions during home visits and could come across unpleasant body odours, dust, noise/ Body fluid or even verbal aggressions
* Evidence of and commitment to multi- oprofessional working.
* Understanding of the interface between primary and secondary care
* oComprehensive understanding of the roles of others in health and social care


Knowledge

* Literate in IT/Computer Skills
* Evidence of knowledge of clinical governance
* Knowledge of the principles of Safeguarding of Vulnerable Adults and Children.
* Knowledge of the principles of health promotion and prevention of illness.
* Good oral communication skills based on fluency in English language
* oDemonstrate an understanding of national government initiatives in health care.


Freedom to act

* Work within clearly defined clinical and professional policies
* Ability to work independently seeking appropriate support and advice as necessary
* Works within codes of practice and professional guidelines

* Current UK driving license and access to a car
* Ability to travel across sites and across Trust boundaries to meet patient needs and attend meetings, etc.
