Overview
We are seeking a dynamic, motivated and innovative Occupational Therapist to join our integrated multi-professional, national award winning Stay Well At Home service within Cwm Taf Morgannwg University Health Board. The Occupational Therapist works closely with the Physiotherapist, Social Workers and Support Workers to provide a rapid response service to people attending the Emergency Care Department, Clinical Decisions Unit, Acute Medical Unit and Wards. The overall aim of the service is to avoid unnecessary hospital admissions and support timely discharges using a discharge to assess model.
This is a 7 day week service and staff are expected to work weekends on a rotational basis – 3 x 12 hour shifts per week (with an additional 6 hours makeup shift every 4 weeks). Hours of work 07:30 – 20:00.
Responsibilities
* Provide a rapid response service in collaboration with the multidisciplinary team to patients in the Emergency Care Department, Clinical Decisions Unit, Acute Medical Unit and Wards.
* Work to avoid unnecessary hospital admissions and support timely discharges using a discharge to assess model.
* Collaborate with colleagues in the Stay Well At Home Team, including Physiotherapists, Social Workers and Support Workers.
* Operate within a 7 day week service with weekend shifts in rotation; typical hours 07:30 – 20:00 (3 x 12 hour shifts per week with a 6 hour makeup shift every 4 weeks).
Qualifications & Requirements
* Welsh Skills Desirable: Speaking & Listening at Level 3 (equivalent to CEFR B2). Fluency not required; basic conversations with patients about their everyday health are expected.
* Excellent clinical reasoning, time management, observation and communication skills.
* Team player who enjoys working in a fast-paced environment and demonstrates a genuine interest in achieving wellbeing and positive health outcomes for the population served.
