Are you excited and motivated by the personalised care agenda, and ensuring patients have access to the right support at the right time, in the right way? We are seeking exceptional personnel to join us in delivering best practice personalised care and support interventions to cancer patients and their families as part of our Living with and Beyond Cancer Program at Sandwell and West Birmingham Hospital NHS Trust.
Responsibilities
* Work with Cancer Services and the Cancer Nurse Specialist Team to serve as a primary point of contact for cancer patients and their families, helping them navigate their cancer pathway from diagnosis to follow‑up.
* Provide reassurance, telephone triage, support, patient‑related information, and signposting.
* Assist CNS teams with administrative duties related to nurse‑led clinics, MDT outcomes, and tracking of patients on remote surveillance.
* Deliver Holistic Needs Assessments and Care Planning telephone clinics to support patients in resolving non‑complex needs and concerns.
* Manage patients on patient‑stratified follow‑up through remote monitoring.
* Collaborate with the Living with and Beyond Cancer team to embed and integrate personalised care into cancer pathways and participate in health and wellbeing events.
