You will also be responsible for reviewing the current model of our Day Service provision with a key focus on promoting independence and engagement within communities. The current service supports 82 people with complex needs across 5 sites.
These reviews will define an optimum delivery model to inform commissioning decisions.

This is an exciting opportunity for an enthusiastic and motivated transformational Manager to lead these crucial reviews. Key to all our services for Adults is supporting people to live well at home within our integrated health and care economy. Underlying this is a need to develop and deliver services within the available resources.
Adult Services is prioritising the early support agenda, work on our carers offer, housing and accommodation development, support to live well at home, the development of employment opportunities and skills and expanding wider opportunities for individuals within community based settings.
The successful candidate will be directly supervised by the Head of Service for Learning Disabilities & Autism and will be an integral member of our Adult Leadership Team.
