Paramedic Practitioner - Bank - Home Visiting Service
Channel Health Alliance, a GP Federation providing NHS Services isseeking to recruit Bank paramedic practitioners and paramedics. The role is due tothe delivery of the home visiting service to support primary care that ChannelHealth Alliance provides across all localities within the South Kent Coast area.
This post will work clinically seeing patients in their own place ofresidence on behalf of the GP practices who triage and identify patients who meetthe service criteria. The post holder will ensure the smooth running, clinicalquality and safety of the service.
The post holder will have theskill set to maintain high standards of holistic and comprehensive clinicalassessment of patients with a variety of conditions supporting diagnosis andsupplying medication as appropriate. For those who require any further trainingspecific to the job role will provided by the Company. The post holder willwork collaboratively across the service to meet the needs of patients togetherwith colleagues internally and externally. Providing support to the GPpractices and other members of the service where appropriate. The post holderwill deliver care within the boundaries of their role and competence.
The successful candidates will have a good understanding of the healthand social care system and whole system delivery. You will have evidenceof continuing professional development.
Reimbursement of mileage @45p per mile
Main duties of the job
To assess, diagnose, treat,refer or signpost patients/service users who require a home visit withundifferentiated or undiagnosed condition relating to minor illness or minorinjury.
Use advanced clinical skills toprovide education to patients, promoting self-care and empower them to makeinformed choices about their treatment.

Job responsibilities

To provide high qualityholistic patient care and treatment
Supply medications asappropriate following policy, patient group directives and local pathways.
To advise and support juniormembers of the team in the care, treatment and anticipatory care planning forpatients.
Participate in settingstandards for care and auditing clinical outcomes.
Identify and complete necessaryinterventions, within scope of practice and following guidance from existingpolicy and procedures.
When necessary refer tofacilitate further advanced assessment and guidance from the wider team.
Work closely with other careproviders and agencies to ensure quality and safety.
Refer any safeguardingconcerns, using established safeguarding referral pathways.
Accurately and professionallydocument all patient contacts, following all relevant organisational policy andprocedure.
Know own limits in relation toscope of practice and recognise when a requirement exists to refer to, or seekadvice from a more senior clinician.
Ensure compliance withorganisational policy and procedure in relation to information sharing, patientconfidentiality and other relevant policies.
Take responsibility for thesafe storage and administration of drugs and equipment.
Provide care in the patientshome by holistic assessment, implementation and evaluation of programmes ofcare as appropriate.
Participate in setting andmonitoring standards to ensure effective care and delivery of evidence basedpractice.
Involvement in team discussionsto ensure best practice.
Ensure that professionalstandards are maintained and within the guidance by the Department of Health,the Health and Care Professions Council (HCPC) and the College of Paramedics(COP)
Person Specification
Qualifications
* Paramedic qualification and HCPC Registration/ registered clinician & registered with regulatory body
* Diploma in Health Care Practice or evidence of recent HEI study
* Evidence of CPD
Experience
* Experience of working across a variety of multidisciplinary teams
* Operational experience of supporting patients with acute symptoms
* High level of communication and interpersonal skill
* Autonomous Clinical Decision making
* Confident and persuasive presentation skills
* Able to work collaboratively with others and within a team
* Competent and skilled paramedic/clinician
* Highly motivated to provide and engender the highest standard of professional practice
* Able and willing to challenge practice
* Respected by colleagues / peer group
* Computer/IT literate
* Knowledge of pre-hospital care and able to demonstrate credibility
* Minimum of 3 years post registration experience
* Knowledge and understanding of current health and social care agendas
* Knowledge and understanding of community based care provision
* Experience of telephone patient assessment/contact
* Clinical Audit experience
* Knowledge and experience of commissioning within NHS

* Willing to move locations if required
* Clean driving licence with access to a fully insured car as required for business purposes
