Overview
The Asthma Nurse is a Band 6 Respiratory Nurse and the clinical expert and professional resource to the multi-professional team, patients, and carers within their Speciality Service. This is a new service delivered by practices within the Dacorum locality on behalf of Delta PCN. The post involves advanced clinical assessment, evidence-based respiratory care, patient advocacy, audit and education, in a primary care setting.
Responsibilities
* Develop a high level of clinical decision making, monitor and improve standards of care, and work within an advanced scope of professional practice.
* Clinical care by managing a patient caseload, providing expert assessment, planning and evaluation.
* Manage nurse-led clinics (within Speciality/service) and contribute to multi-professional management of patients in the specialist area, including ongoing care needs and discharge planning.
* Support patients and their families with information to enable self-management and participation in care delivery.
* Encourage the multi-disciplinary team to resolve patient problems through innovative models of case management.
* Establish and maintain effective communication with patients and carers/relatives and refer to other services as appropriate.
* Where appropriate, undertake a non-medical supplementary and independent prescribing qualification to prescribe within the area of specialty.
Leadership
* Act as a role model and provide specialist clinical knowledge to clinical colleagues through MDT meetings.
* Undertake clinical supervision of nursing colleagues and act as a resource for health care professionals.
* Support service development, implement change, and promote safe, high-quality care.
* Lead on risk assessment, incident reporting, and promoting a culture of openness and accountability.
Advocacy
* Maintain high standards of nursing care and involve patients and carers in planning and delivery of care.
* Advocate for patient confidentiality, privacy, and cultural sensitivity; develop patient-focused education strategies within the specialty.
Clinical Governance / Quality
* Ensure compliance with policies, procedures and clinical guidelines; monitor and improve standards of care through audits and research.
* Promote evidence-based practice, disseminate good practice, and involve patients and the public in service improvement.
* Promote equality, diversity, and rights; identify and address clinical issues affecting quality of care.
Education / Training
* Take responsibility for ongoing professional development and participate in supervision and teaching of staff.
* Lead educational programmes related to the specialty and ensure the clinical environment supports learning.
Professional Responsibility
* Maintain active UK professional registration; comply with NMC code of conduct and revalidation standards.
* Maintain awareness of current research and participate in professional forums and governance initiatives.
* Ensure patient safety and promote a safety culture; contribute to personal development plans and mentorship for nurses.
Qualifications / Skills
* Essential: Registered Nurse / Midwife with degree or equivalent; respiratory qualification; ARTP accreditation for spirometry; ability to perform FeNO testing and interpretation; experience in COPD management.
* Desirable: Change management principles; Root Cause Analysis.
Experience
* Essential: Broad clinical experience with ability to translate learning into practice.
* Desirable: Experience of managing patients with respiratory disease in any setting.
UK Registration
Applicants must have current UK professional registration.
