We are looking for an ambitious, dynamic, motivated and enthusiastic Band 6 caseload holder or qualified District Nurse to join our Community Nursing team in Mid West Beds. This highly respected team is based in Steppingley Hub and delivers responsive, high quality nursing to patients in their own homes. Main duties of the job

We are looking for an experienced community nurse, looking for the next step in their career, community nursing services are expanding in line with the 10 year plan to provide more care in the community, this is an exciting place to be at this time. We are seeking a motivated and passionate Senior Nurse to support the community teams to deliver person centred, research based care. You will have the skills and experience to manage your own caseload, which will be aligned to the GP practice and residential homes within the area. You will be working with the Mid West team, covering Ampthill, Flitwick, Barton, Woburn and surrounding villages. Excellent communication and time management skills, and sound clinical knowledge, are essential skills. A flexible approach is required to accommodate the changing needs of the service along with the drive to influence service development.

Detailed job description and main responsibilities

You must have a driving license and have access to a vehicle in order to carry out your duties. You will be required to work a 7.5 hour shift pattern covering 7 days a week.

This is a fulltime post - 37.5 hours

Person specification

Education

Essential criteria

1. Registered General Nurse
2. Demonstrate evidence-based practice and up-to-date clinical skills
3. Evidence of significant post registration nursing experience including community setting
4. Post-registration qualification in a relevant clinical field
5. Relevant teaching/mentorship qualification and evidence of teaching skills.
6. Post-registration nursing experience, including community experience

Desirable criteria

7. Specialist Practitioner Qualification
8. Nurse Prescriber

Other

Essential criteria

9. Current driving licence
10. Ability to travel across sites and across Bedfordshire to meet patient needs and attend meetings, etc

Experience

Essential criteria

11. Evidence of and commitment to multi-professional working
12. Understanding of the interface between primary and secondary care
13. Comprehensive understanding of partnership working
14. Ability to manage a caseload
15. Be able to work in collaboration with patients, carers and the MDT

Desirable criteria

16. Understanding of safeguarding and essentials of managing complex cases
17. Be a proactive nurse with a strong desire to lead on the delivery of quality, holistic care
18. Be a positive role model and mentor to support junior staff in their day to day clinical activities

Knowledge

Essential criteria

19. Literate in IT/Computer Skills
20. Knowledge of the principles of Safeguarding of Vulnerable Adults and Children
21. Knowledge of the principles of health promotion and prevention of illness
22. Good oral communication skills based on fluency on the English language

Desirable criteria

23. Demonstrate an understanding of national government initiatives in health & social care & how they impact on local services
24. •To utilise systmone data and other performance and activity data to influence practice and improve local service delivery.
