The Frailty Unit provides specialist multidisciplinary care for older people living with frailty, focusing on early assessment, rehabilitation, and safe discharge planning while applying home first principles. You will work within a collaborative team, benefit from ongoing professional development, and contribute to service improvement initiatives that enhance patient outcomes.
As a member of the multidisciplinary team, you will have responsibility for assessment of care needs and the development of programmes of care, and/or the implementation and evaluation of these programmes ensuring the delivery of high-quality care to patients. You will be expected to carry out all relevant forms of care without direct supervision and may supervise registered and unregistered staff, as well as student nurses, as needed.
Full- and part-time hours will be considered.

Qualifications
* Registered Nurse with valid UK NMC Registration
* Excellent teamwork and the ability to work using your own initiative
* Effective listening, communication and interpersonal skills
* Ability to work with people and as part of a multidisciplinary team
* Excellent time management skills
* Working knowledge of basic information technology
