Join Our Integrated Leadership Team Group Manager – Bedded Assessment and Reablement We’re recruiting a Group Manager to lead our Community Support Bed and Pathway 2 Discharge Teams within Derbyshire. These Teams form part of Derbyshire’s integrated bedded short-term assessment and reablement services. They provide local people access to assessment, reablement and rehabilitation support that maximises their own skills and abilities, even after a hospital or medical episode. This is commonly known as “intermediate care”. You would be overseeing 3 multi-disciplinary Community Support Bed settings in Derbyshire (Bennerley Fields, Staveley Centre, and Thomas Fields) and the P2 Discharge Team who complete social care assessments and help people to plan for home from Community Support Beds and Community Hospitals across Derbyshire. You will be responsible for the development of and operational delivery of these teams which include regulated services and deliver prevention, assessment and support planning duties under the Care Act. You will ensure compliance with statutory and regulatory standards. You will drive and deliver quality standards, maximise effectiveness and efficiency and ensure that the services are fit for purpose both immediately and for future requirements. You will deliver on future strategic plans and implement change as required. Derbyshire County Council and Derbyshire Community Health Services have agreed to integrate a range of services that support people to remain at home when their needs change, but also to return home following a hospital admission by receiving shot term assessment, rehabilitation and reablement support. Both organisations are already working together to streamline health and care support for local people and we are now at a stage where we are pushing forward to achieve a greater level of “joined up” care. The objectives of our partnership are attached for further information. You will lead your teams to: Ensure more people are supported to remain at home independently following changes in their health or care situation Take a strength-based approach and work in a way that makes the most of the person’s abilities and strengths to gain the best outcomes for them Ensure your teams are high performing and able to respond flexibly, which will include transforming practice. Collaborate with a range of professional teams and leaders to deliver the required service outcomes and increase the productivity of our teams. Provide a consistent intermediate care offer, with more people supported to be able to manage for longer at home. Reduce and improve processes so that individuals and their families only have to “tell it once” and have a streamlined experience.
