Overview
Waterside Unit is a 21 bedded ward within the Integrated Elderly Care Division. We provide inpatient rehabilitation for older adults working closely with our therapy team until ready for discharge home with support or a placement in a Nursing or Residential Home. Our patients often have multiple health needs & chronic conditions.
Waterside Unit is part of Amersham Hospital, one of several Community Hospitals within Buckinghamshire Healthcare NHS Trust. The Trust covers Buckinghamshire and surrounding areas and employs over 6,000 clinical staff across many disciplines.
Responsibilities
* Identifying the needs for nursing care of individual patients in the context of prescribed medical care.
* Undertaking the nursing assessment of the patient, including risk assessments (e.g., tissue viability, falls) and re-evaluating findings.
* Planning and delivering an individual programme of nursing care for each patient, with objectives, and re-evaluating as needed.
* Supervising qualified and unqualified staff to ensure the optimal application of skills necessary to provide the planned care.
* Recording nursing care given and evaluating outcomes in relation to objectives; modifying care plans accordingly.
* Acting as a health educator throughout the patient’s stay.
* Responding sensitively to the needs of patients’ families and visitors.
Qualifications / Experience
* Band 5 Nurse with current registration and a supportively structured pathway for newly qualified nurses or those due to qualify soon.
* Interest or experience in elderly care and inpatient rehabilitation is desirable.
