As a Support Worker in one of our supported accommodation homes, you will play a vital role in providing holistic care, guidance, and support to young individuals transitioning to independent living. Key Responsibilities
1. Develop and maintain personalised support plans aligned with LA Care standards. Keep them updated to reflect current needs and goals.
2. Deliver key work sessions in line with support plan goals through one-to-one education and reflective conversations.
3. Provide tools, guidance, and referrals to external agencies to help young people achieve their goals. Work with young people to identify education and career aspirations, connecting them with appropriate resources.
4. Maintain accurate records of progress, behaviours, and interactions, adhering to confidentiality and reporting requirements within the outlined timeframe.
5. Complete daily checklists, cleaning tasks, and review communication logs.
