Key Responsibilities
* Be part of a new service model delivering high quality Care Act statutory functions in relation to new Mental Health Adult Social Care referrals.
* Contribute to meaningful service improvement and change.
* Work in a system where partnership working is critical and valued.
* Undertake social care assessments and reviews that promote independence, improve well‑being, and ensure outcomes are being met.
* Produce and agree a care and support plan, if required, with individuals and their carers/families which acknowledges the strengths within their current situation and any additional support that will enable them to remain as independent as possible.
* Encourage individual choice and participation in service planning and ensure that the person's needs are understood by service providers.
* Help resolve conflicts between individuals, carers and/or services.
* Develop knowledge of community‑led support, building relationships with service providers and voluntary groups to ensure people can access and/or purchase support to meet their desired outcomes.
* Support individuals to identify and utilise their informal network of support.
* Calculate personal budgets and explore Direct Payments as part of the assessment of need.
* Contribute to shaping and influencing the transformation of Mental Health Adult Social Care in Salford as part of a new Mental Health Social Work Service.
* Work within a system that values partnership, innovation and continuous improvement.
* Contribute to wider transformation across Adult Social Care.

Key Qualifications
* Has experience working within statutory mental health services and/or Adult Social Care.
* Brings strong knowledge of the Care Act, Mental Health Act, Mental Capacity Act and associated frameworks.
* Has a proven track record engaging well with individuals experiencing a range of mental health conditions, and their carers and families.
* Is committed to person‑centred, strengths‑based and trauma‑informed approaches.
* Has experience forming productive working relationships with a range of professionals within multidisciplinary settings.
