We're working with a well-established Health & Social Care Partnership in Falkirk to recruit an experienced Social Worker into a fast-paced Hospital Discharge Team — where you'll play a key role in supporting safe, timely discharges and improving outcomes for vulnerable adults.

The Role
You’ll be part of a dedicated Hospital Discharge Team, working at the heart of the hospital to ensure individuals are discharged safely with the right support in place. This is a hands‑on, fast‑paced role where your assessment skills and ability to coordinate care will be key.
Key Responsibilities
* Complete strengths‑based assessments to support safe discharge planning
* Work closely with patients, families, and carers to develop appropriate care plans
* Assess capacity and risk, ensuring decisions align with legislation
* Collaborate with multi‑disciplinary teams (nurses, doctors, AHPs)
* Arrange care packages, placements, and community support
* Ensure smooth transitions from hospital to community services
* Maintain accurate, timely case records
What we're looking for
* Qualified Social Worker
* Registered with the Scottish Social Services Council (SSSC)
* Recent experience in adult social work (hospital discharge or community ideal)
* Strong skills in assessment, care planning & risk management
* Confident working in a fast‑paced MDT environment
* Excellent communication and organisational ability
