We are transforming Adult Social Care to meet rising demand, with four Locality teams aligned to the PCN footprint (Wallington, Carshalton, Sutton & Cheam). Locality teams bring Social Workers together with NHS colleagues to support holistic, integrated working.
About the Role and Our Ideal Candidate
Join Adult Social Care as a Social Worker within the Localities team to empower adults to live independently. You will contribute to crafting outstanding care plans that prioritise service users’ needs and voices, integrating input from families/carers and external agencies to achieve the best outcomes. This role supports ongoing professional development and supervision within a welcoming, supportive, and energetic team.
Responsibilities
* Undertaking Care Act 2014 Assessments – identify individuals’ care and support needs with a personalised approach
* Completing Care Act 2014 Reviews – evaluate the ongoing suitability of care and support plans
* Managing safeguarding enquiries – investigate concerns to ensure safety and well-being of vulnerable individuals
* Mental Capacity assessments – determine an individual’s ability to make specific decisions
* Undertaking duty on rota basis
* Undertaking DSTS and CHC
* Court-related work
Our teams are known for crafting care plans that involve service users and coordinate with families, carers and external agencies to achieve the best outcomes.
This position is suitable for individuals with strong communication skills and a compassionate manner, capable of managing high levels of emotional distress. Requirements / Qualifications
* Registered with Social Work England and hold a degree in Social Care or a related field
* At least 2 years post-ASYE UK registered social worker experience
* Experience working in a multidisciplinary team
* Experience in complex mental health and physical health cases
* Strong knowledge to deliver social work support to service users and their families
* Ability to communicate complex social issues clearly to patients and professionals
* Ability to plan and coordinate social care plans and aspects of patient discharge
