We are looking for a Health and Social Care Coordinator to join our Torbay Adult Health and Social Care team for a permanent 30‑hour position (Monday to Friday, 8am‑6pm, with occasional weekend and bank holiday work).
Responsibilities
* Provide a single point of contact for community health and social care referrals, communicating with a wide range of stakeholders over telephone, email, and face‑to‑face.
* Collate and record needs‑led referrals or assessments, applying Care Act eligibility and identifying potential risk.
* Give advice, information and signpost to local statutory and voluntary services; complete full Care Act assessments of need and formulate Support Plans.
* Act as a keyworker, managing a caseload and adjusting support plans as required.
* Coordinate a caseload working with a multi‑disciplinary team; commission appropriate services and liaise with service users, carers, families, and colleagues.
* Use IT solutions and assistive technology where suitable, ensuring compliance with GDPR and other relevant legislation.
* Advise on the financial assessment process and ensure referrals are made to the financial assessments and benefits team.
* Maintain mandatory training records and contribute to multi‑disciplinary assessments in line with the Care Act 2014.
Qualifications
* Excellent communication and IT skills.
* Knowledge and understanding of Care Act legislation and eligibility criteria.
* Experience working in a health and social care setting with patients/service users and the public.
* Ability to write clear, concise documents and reports.
* NVQ Level 4 in Health and Social Care or equivalent (required to progress to band 5); enrolment for this qualification will be available within the role.
