We are excited to offer a rewarding opportunity within the Inverness District Flow Hub for a Band 4 Health and Social Care Coordinator who will be part of the Inverness Integrated Team.
Overview
The HSCC plays an integral role within the Integrated Team, supporting the provision of person‑centred, safe and effective delivery of care. The role involves managing referrals from professionals, patients, and members of the public, collating information from a variety of sources to support early multidisciplinary team decision‑making and prioritise delivery of care and services.
Responsibilities
* Manage referrals from professionals, patients, and members of the public.
* Collate information from various sources to support early multidisciplinary team decision making.
* Assist in prioritising and utilising service delivery appropriately.
Qualifications and Skills
* Excellent communication, interpersonal and organisational skills.
* Health and Social Care qualification or experience in the sector; strong understanding of how health and social care is delivered.
* Competence with computer systems and attention to detail for accurate record‑keeping.
Eligibility
