The Role
It is a very challenging time for Adult Services and it is essential that we support people to maximise their independence as much as possible and in a timely way. If you are someone who has a passion for strength-based conversations that results in person-centred outcomes then we would like you to express your interest in the role of Social Worker in the review team.
This is an opportunity to influence and shape what services we need to effectively support our service users. You will put plans in place which encourage people to live healthy and independent lives and have the ability to be innovative in the development of those plans.
You will be part of the Quality Assurance & Improvement section of the department reporting into a Social Work Deputy Team Manager who will provide you with reflective and case load supervision to support your continued professional development. Please note this is a part-time post.
About You
Through an assessments/review process you will develop strength-based support plans in line with the Care Act and Mental Capacity Act and other relevant legislation/best practice. You will link people into a range of preventive and independence services such as universal and voluntary, utilising technologies and equipment with the aim of positive risk taking and thinking ‘outside the box’.
As part of the team you will need to be able to work flexibly and respond to changes and direction in the teams focus dependant on the needs of the service. You will be working with all service user groups at the point of review.
