What you will do:

* Complete comprehensive assessments and reviews, including Decision Support Tool (DST) assessments, and present findings to the panel.
* Work both in the office environment on Wednesdays and in the community, where you’ll need access to transport to reach clients and other professionals.
* Collaborate with a multidisciplinary team, sharing insights and strategies to enhance patient care and support.
