Assistant Practitioner for Falls Prevention (RLH Site) Band 4 (Annex 21)

Are you an experienced and qualified Assistant Practitioner Band 4 with current NHS hospital experience, preferably on the University Hospitals Liverpool Group (UHLG) sites? Have you been a falls champion? Do you want to work within a team of highly experienced clinical staff?
We are seeking to appoint 1 new band 4 Assistant Practitioner who can support our clinical staff in preventing falls. The post will involve developing relationships with staff in all areas of the trust, educating staff at all levels, providing clinical care to patients and advice to our staff. You will be fully supported by the Falls team to undertake your role. It is anticipated that your day‑to‑day view of the role will involve working in the clinical areas, advising clinical staff on appropriate prevention strategies, providing on ward and off ward education in relation to falls prevention. You should be confident in providing written and verbal information on prevention to patients, staff and relatives.
Main duties of the job
Specific duties:
* Work under an agreed framework determined by the falls lead practitioner (registered professional) and proximal supervision of the falls team registered professional.
* Review patients allocated by Falls Nurses/Practitioners.
* Provide falls prevention training for clinical staff, including bank and pool staff.
* Attend the Monthly UHLG Falls Steering Group.
* Contribute to the monthly Falls team meeting.
* Participate in ward‑ and team‑based falls prevention quality improvement projects.
* Act as a mentor for falls champions, students, cadets and HCAs.
* Provide ward‑based staff education on falls prevention strategies.
* Conduct clinical auditing involving deep dives and patient and staff experience questionnaires.
* Participate in Falls Awareness weeks/days to promote falls across the trust.
The post will primarily be based on the Royal Liverpool Hospital site; however, you must be willing to work on any site if the service delivery requires this at any point. If you are interested or just want to know more, please contact us for an informal visit or chat.

* Foundation degree in assisting professional practice/equivalent experience
