Overview
CityView Primary Care Network (PCN) has an exciting opportunity for a Social Prescribing Link Worker to join our team. We are looking for someone to work between 16-24 hours per week, based across the 5 practices within our network. We are seeking a friendly, enthusiastic, and compassionate individual with excellent communication skills and the ability to work confidently with a diverse range of people.
Responsibilities
* Receive referrals from GP practices and a wide range of agencies
* Co-produce personalised care and support plans with patients
* Provide one-to-one support through consultations, phone calls, and community visits
* Build strong links with local VCSE organisations and community groups
* Promote social prescribing across the PCN and educate staff on its value
* Maintain accurate records and ensure appropriate coding of referrals
* Support patients to access services and attend activities, including accompanying them when needed
* Encourage patient feedback and contribute to service evaluation
* Collaborate with the wider PCN team, including the Occupational Therapist
* Attend team meetings and provide updates on social prescribing activity
Support & Supervision
* Regular clinical supervision from a named GP or clinician
* Day-to-day support from the Practice/PCN Manager
* Peer support from other Link Workers across the PCN
* Access to ongoing training and CPD opportunities
Person Specification
Experience
* Experience in primary care, community, social care, or voluntary sector
* Excellent communication and interpersonal skills
* Ability to work confidently with a diverse range of people
* Commitment to reducing health inequalities
* Ability to travel across the local area
* Experience working in General Practice or multi-disciplinary teams
* Familiarity with SystmOne or similar systems
* Knowledge of local VCSE services
* Training in motivational interviewing or coaching
* Understanding of the wider determinants of health
