Overview
The post holder will be responsible for the leadership and development of specific Community Services, supporting effective working across the interface of community teams, clusters and secondary care, social care and the third sector. The role will oversee the Hospital@home service and community discharge teams. As a key member of the Primary and Community Care Group leadership team, the focus is to support operational delivery with team managers and clinical leads, embedding the community transformation agenda and new ways of working as business as usual. The post holder will lead the delivery of integrated community services with a focus on safe, effective, and person-centred care and will develop efficient pathways to support patients across the wider system.
Responsibilities
* Provide leadership and development of diverse community services and related functions.
* Support effective working across community teams, clusters, secondary care, social care and the third sector.
* Oversee Hospital@Home and discharge services.
* Lead operational delivery with team managers and clinical leaders; embed the community transformation agenda and new ways of working.
* Ensure safe, efficient, and person-centred care; develop efficient pathways to support patients across the wider system.
* Maintain managerial responsibility for operationalisation of community transformation plans, delivering quality, performance and financial targets in line with organisational aspirations.
* Collaborate with the Senior Management Team to translate strategic direction into operational practice for Hospital@Home and Discharge Services.
* Contribute to service redesign and provide guidance to the Directorate Manager and Leadership Team.
* Work with Integrated teams and professionals to ensure consistency of approach and seamless pathways of care.
* Ensure a clear, accessible referral system and monitor its effectiveness through the performance framework.
* Monitor and maintain high standards of health and care service delivery; provide innovative solutions to improve service quality.
* Regularly review service plans and adjust for local needs and national policy changes.
