Essential:
1. Band 5 or above.
2. Must have a valid driving licence and access to a vehicle.
3. Registered nurse with a current NMC pin.
4. Relevant Nursing qualifications.
5. Hardworking RGN/ RGN.
6. 6 months recent experience working as a Community nurse.
7. 2 clinical references that cover 3 years work experience.
8. Valid DBS certificate.

Responsibilities:
1. Carry out care plans in collaboration with patients, families, and multidisciplinary teams.
2. Provide skilled nursing care including wound care, medication administration, and symptom management.
3. Co-ordinate care and liaise with other healthcare professionals and community services.
4. Maintain up-to-date documentation of assessments, care plans, and interventions.
5. Excellent clinical assessment and communication skills.
