Health Care Assistant post : Hours maximum of 27 hours per week ,over 3 days.

To assist the GPs,AHP,ANPs and Practice Nurseteam in the service delivery and management of patients.

To abide by practice policies and procedures.

Always maintain patients privacy and dignityand provide a level of care appropriate in a compassionate and sensitivemanner.

Be part of a forward-thinking practicecontributing and assisting in areas of audit and evidence-based practicedevelopment.

Develop and maintain nursing competencies inphysical assessment skills to maintain high quality and safe care.

The post holder will be expected to contributeto general house-keeping duties within the practice, such as general tidiness,maintain infection control standards, re-stock levels are met and maintainedand any clinical equipment that is used is clean, working and stored correctly.

Assist with admin and clerical duties asrequired.


Main duties of the job

Always ensure privacy and dignity.

When relevant, understand why observations aretaken and be able to take and record appropriately. Inform nurse or GP or anyconcerns or above target recording as appropriate and by specific policyguidance.

Safeguarding vulnerable adults and children byreporting any concerns to Safeguarding leads within the practice.

Be competent and capable of providing physicalassessment as required by requests of PN/GP/ANP.

To be able to complete competently therequirements of the QOF templates in LTC (Long term condition) management.Including foot checks, venepuncture, BP, Height, weight etc.

Vaccinate children and adults within the remitof PSDs as required by immunisation policy.

Administer safely B12 IM injections disposing ofequipment as appropriate as per guidance.

Assist as required in the emergency managementof patients.

Assist within minor surgery / Coil clinics asrequired.

Provide relevant patient information leaflets tothe patient.

Ensure that all PNs,GPs,ANPs are kept informedof any changes with the patients conditions or observations as guidance or asrequested.

Job responsibilities

Monitor and maintain health, safety and securityof self and others.

Will carry out delegated, repetitive, routine,and familiar tasks.

Will carry out routine elements of assessment toenhance the patient journey assist in the care provided by the nursing and GP/ANP team.

Person Specification


Qualifications

* GCSE Grade 4 C or above English and Maths
* Ability to communicate with members of the public and health care providers.
* Presents self as courteous, respectful and helpful
* Ability to work effectively under direct supervision, and as part of the whole practice team.
* NVQ Level 2 in Healthcare or desire to work towards
* Care Certificate
* Use of IT software such as Word, Excel, Outlook
* Experience of caring in a Primary Care setting
* Venepuncture
* Competence in physical skills e.g. ECG, BP etc.
* An understanding of working within Primary Care.
* Full training and support will be provided to the successful candidate as needed.
