Summary
We are hiring an LVN.
Responsibilities
1. Provide high-quality clinical services within the scope of practice, as defined by state laws, in accordance with the plan of care, and in coordination with other healthcare team members, following all infection control procedures. Do not alter the plan of care.
2. Continuously observe and report the patient's nursing needs to the supervising RN.
3. Communicate effectively and timely with physicians, patients, family members, and agency staff regarding patient care issues, visit schedules, changes, orders, OASIS data sets, coding requests, and coordinate care to ensure services are delivered as ordered and in compliance with laws and regulations.
4. Participate actively in performance improvement initiatives, aiming to reduce hospitalizations and improve patient outcomes and processes.
Education and Experience
* Current LVN licensure in Texas.
* Valid Driver's License, vehicle insurance, and access to reliable transportation.
* CPR Certification required.
