Join our team as a Wellbeing Care Co-ordinator in North & West Herefordshire Primary Care Network.

Do you want to make a real difference by supporting independence, mental wellbeing, and social connectivity?

Were looking for an enthusiastic Wellbeing Care Co-ordinator to join our friendly team and be the first point of contact for patients needing proactive well-being support across five local practices.

You will work closely with GP teams, Care Coordinators, social prescribers, health professionals, and community groups to help patients get the right support for their needs.

Youll also be involved in local health projects that focus on helping people who may find it harder to access services.

Working alongside our supportive Wellbeing Team, youll help individuals and groups by offering personalised care guiding them to make informed choices that support their health and wellbeing goals.

Main duties of the job

In this varied role, youll support patients to access the care and services they need while promoting health and wellbeing across the PCN.

You will coordinate new referrals to the Wellbeing Team, offer advice and signposting, and work one-to-one with patients to help them take an active role in their care. The role also includes supporting group programmes for long-term conditions, engaging in community outreach, and promoting the Wellbeing Team across GP practices and communities. You will work closely with GPs, social prescribers, health coaches, dietitians, and safeguarding leads.

We are looking for a reliable, people-focused person with strong communication skills, able to manage a varied workload, record patient interactions accurately, and collaborate effectively. An understanding of health inequalities/ interest in community health initiatives is essential, with a willingness to learn and develop through training and supervision.

Job responsibilities

The Wellbeing Team work with individuals in need of proactive support to encourage independence, a health lifestyle, mental wellbeing and social connectivity. They work with General Practices and their communities where there are identified health inequalities to engage and support people in accessing health and community services

The Wellbeing Care Coordinator (WBCC) has a key care coordination role within the N&W PCN Wellbeing Team in supporting people to live independent, healthy and positive lives. The role will:

* Act as a first point of contact for all new referrals to the Wellbeing Team and the single point of access for staff and patients supporting referral issues, early signposting advice and queries.
* Involve working one to one with patients who need extra support and time, helping them to be actively involved in their care and directing them to the most appropriate services based on what is important to them.
* Support the wider Wellbeing and Primary care team in delivering specific health inequality initiatives across the PCN. This could include promoting uptake to screening services, coordinating self-management group based programmes to manage long term conditions such as BP, diabetes and community outreach initiatives working alongside public health teams
* Actively engage with practices to promote the activities and support available from the Wellbeing Team across all practices
* Work closely with and understand the different roles within the wider Wellbeing team (Social prescribers, Health and Wellbeing coaches, dieticians, Care Home and Safeguarding coordinators.) and General Practice based staff.

There will be a need to work agilely- within the local community and in practices as well as remotely depending on the requirements of the service.
