This is an excellent opportunity for a qualified ACP who is passionate about acute and ambulatory medicine and thrives in a fast-paced, multidisciplinary environment focused As An ACP In Medical SDEC, You Will Undertake autonomous assessment, diagnosis, and management of patients presenting with acute medical conditions suitable for same-day care Formulate and implement management plans, including prescribing and requesting/interpreting investigations Work collaboratively with consultants, junior doctors, nursing staff, and the wider MDT to facilitate timely decision-making and safe discharge Contribute to service development, quality improvement, and education within SDEC Support patient flow and help reduce unnecessary admissions while maintaining patient safety and experience You Will Be A qualified Advanced Clinical Practitioner with a completed MSc (or equivalent) in Advanced Clinical Practice Registered with the relevant professional body (NMC / HCPC) An independent non-medical prescriber Clinically confident in managing undifferentiated acute medical presentations Experienced in acute medicine, emergency care, or SDEC/ambulatory care settings A strong communicator with the ability to work autonomously and as part of a multidisciplinary team Main duties To be competent in the assessment and treatment of patients presenting with a range of undifferentiated and undiagnosed illnesses underpinned by extensive knowledge of a range of chronic and acute conditions. To be competent in providing an initial clinical diagnosis and in the decision to refer to an appropriate speciality. To be competent in the ability to discharge patients following appropriate assessment and treatment. To be competent in the initial immediate assessment and treatment of patients with a range of life threatening presentations such as cardiac arrest and acute major illness. If not already an Independent Prescriber, will be expected to successfully complete the programme within a timely manner, in order to continue in the role. To be undertaken and completed within 1 year of qualifying. To be able to see, treat and refer appropriately, a broad range of patients with acute and chronic illnesses as an autonomous practitioner. Administration of medicines, e.g. in line with NMC Guidance and Trust Policy. Demonstrate own personal development with self-directed learning and identifying and accessing internal and external development opportunities, as per individual advanced practice portfolio. Qualifications Essential person specification requirements RGN Level 1 Teaching Qualification Masters level qualification in Advanced Clinical Practice with completed dissertation to achieve Band 8a Qualified Non-Medical Prescriber or to undertake the programme within 1 year in order to continue in the role (V300) Recent and relevant continuous professional development Experience Extensive post registration nursing experience in an acute care setting Experience of working autonomously as an advanced clinical practitioner Evidence of audit and evidence based care Evidence of multi-disciplinary team working Skills & Abilities Teaching, mentoring and assessment skills Management and organisational skills Excellent interpersonal and communication skills Highly developed oral and written communication skills with patients and the MDT; confidence to discuss sensitive, complex patient management and to liaise and negotiate with other members of the MDT Highly developed clinical assessment, analytical and problem solving skills, including generic healthcare aspects to accurately manage complex patient presentations to an excellent professional standard Evidence of ability to critique own practice Ability to manage time and workload effectively and to work in a high pressure environment Self-aware reflective practitioner able to envisage the role, and understand and manage inherent pressures of the role Be part of MY team We are an acute trust caring for over one million people in Wakefield and Kirklees.
