Memory Nurse

An exciting opportunity has arisen for an enthusiastic and committed Band 6 Memory Nurse to join our Countywide EMDASS Team. The post focuses on delivering dementia assessment, diagnosis and post‑diagnostic support, working collaboratively with a multidisciplinary team across West Hertfordshire.
Main duties of the job
The post-holder will undertake memory assessments for service users suspected of having dementia, deliver diagnosis in partnership with the multidisciplinary team, and provide treatment and ongoing post‑diagnostic support through titration reviews. Work will span the One‑Stop Pathway and Primary Care settings.
Risk assessments will be carried out and managed in partnership with service users and carers, following safeguarding policy and protocol. The role includes making tailored referrals to partner agencies and supporting service users to live well with their illness.
The post-holder will act as Duty Nurse for screening referrals, offering advice and signposting.
Supervision of junior staff and trainees, as delegated by the MATS Team Leader, is also required.
Job responsibilities
The post-holder will use appropriate cognitive tools for memory assessment, collaborate with the multidisciplinary team to deliver diagnosis and develop specialist care plans in line with NICE guidelines and evidence‑based practice.
Tasks include: devising care plans that promote independent community living; completing risk assessments with clear management plans; using a range of clinical skills in community and home settings; providing ongoing post‑diagnostic support; arranging discharge with appropriate care provisions and follow‑up; maintaining high standards of record keeping; and contributing to service strategy discussions.
Additional responsibilities cover: supervision and training of students/trainees; diary management; promotion of staff well‑being; reporting fraudulent activity; participating in service transformation; proposing and completing audits; and ensuring adherence to confidentiality and documentation standards.
Person Specification
Qualifications
* Degree, diploma or equivalent in Nursing (Mental Health Nursing).
* Registration with Nursing and Midwifery Council.
* Two years post‑registration experience, with at least one year in older people’s mental health care.
* Post‑registration course in dementia.
* Commitment to become a non‑medical prescriber.
* Understanding of end‑of‑life care in dementia.
* Pre‑ or post‑registration training/short courses in mental health and/or older people’s care.
* Community and inpatient nursing experience.
Experience
* Knowledge of legislation related to mental health and social care (e.g. Mental Health Act, Mental Capacity Act, National Service Framework, National Dementia Strategy, Care Programme Approach).
* Good knowledge of Microsoft Word, PowerPoint and Excel.
* Ability to manage distressing and challenging situations.
* Expert clinical skills in dementia care.
* Experience in research.
* Experience in audit.
