This role is hosted by Royal primary Care at Rectory Road and is joining a professional, skilled and friendly team who work safely with our patients in the Chesterfield area. We want to make a difference to our patients lives by providing evidence-based nursing care.
We provide training and education for the successful candidate and are a team that likes to learn together with regular teaching sessions and information sharing to ensure our standards of nursing are high.

Provide high quality nursing care to our patients in the local community to keep them safe at home, prevent hospital admission, and support management of long term conditions.
You will work closely with the team, which consists of a long term conditions nurse and advanced care practitioner, offering structured, patient-centered care, monitoring, education, and health coaching.
The role will be to assess, plan, implement and evaluate individualised patient care in the most efficient and clinically effective way whilst maintaining the patient’s privacy and dignity.
Implement/refer to the relevant services as appropriate.
Work closely with the Integrated Community Teams, community nursing and local acute Trusts.
Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.
We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people’s own homes and via virtual consultations.
