Overview
As an Intermediate Care Unit with 28 beds, we bridge the gap between home and acute hospital services. Patients who are too unwell or incapacitated in the short term to stay at home but not requiring acute intervention are admitted to our unit to help them recover and return to baseline function. In addition, patients who have had an acute episode but are not yet ready to return home will come to us for a short period of rehabilitation to ensure they are safe to return to their home environment and their abilities are optimised.
We are a team of RNs, Advanced Clinical Practitioners, Healthcare Assistants, Physiotherapists, Occupational Therapists, Rehabilitation Support Workers, Speech and Language Therapist and Ward Doctor who aim to maximise our patients’ potential to enable them to return home. We have a visiting Stroke Consultant and a Geriatric Consultant, both from an acute hospital, who see to and review stroke and medical patients respectively once a week. When that is not possible we work closely with our adult social care colleagues to plan best outcomes.

As this is a 24 hour service, internal rotation will be operating.

Responsibilities
* Lead and support a friendly and collaborative nursing team, applying the importance of prioritisation and assertive leadership.
* Plan the patient pathway, enable progression, and support patients through setbacks to maximise potential for return home.
* Provide input within a 24 hour service, with internal rotation as required.
* Collaborate with multidisciplinary colleagues to deliver high quality, evidence-based care and facilitate staff learning and development.
Qualifications and Experience
* Registered Nurse (RN) who enjoys working as part of a friendly and supportive team.
* Ability to prioritise, lead team members, and work effectively in a rehabilitation setting.
